If You're Considering Breast Augmentation

Breast augmentation, or breast enhancement is our most common requested procedure. Dr. Mabourakh's goal is to meet your expectations. Therefore during your visits you will be asked to discuss your needs and expectations of the surgery at length. Only by reviewing your needs can we proceed to surgery and obtain a result that you will be excited about.

The goals for breast augmentation:

• To enhance the size shape and contour of the breasts..

• To correct volume loss due to weight loss or pregnancy.

• To correct difference in breast size.

• To correct deformities due to breast surgery as in reconstruction following breast cancer

• To correct problems with fitting clothes and bathing suites.

The surgery process involves placement of  silicone or saline implants under the breast tissue or if possible under the chest muscles through a small incision. this incision can be placed in many different location based on your preference and limitations of the breast tissues.

The Best Candidates For Breast Augmentation

Breast augmentation can enhance your appearance and your self-confidence. Before you decide to have surgery, think carefully about your expectations and discuss them with Dr. Mabourakh.

The best candidates for breast augmentation are women who are looking for improvement, not perfection, in the way they look. If you're physically healthy and realistic in your expectations, you may be a good candidate.

Types Of Implants

A breast implant is a silicone shell filled with either silicone gel or a salt-water solution known as saline. Use of silicone implants has been approved by the FDA for almost 2 years. The new silicone implants are much different. They are safer than ever before. The shell has been made thicker to reduce problems with leaking and the silicone gel is also different. the advantages of silicone gel is its look, feel, and shape. Silicone implants are currently more expensive than saline implants but are superior in appearance.

Saline-filled implants have been available since breast augmentation was started. They are still a good choice for those who have any concern about use of silicone implants. Saline implant are less expensive but do not look or feel as natural as silicone implants.

All Surgery Carries Some Uncertainty And Risk

Breast augmentation is an outpatient procedure that is performed under general anesthesia or intravenous sedation. We have a fully approved surgery center in our private office and only use board certified anesthesiologist in our center.

The most common problems encountered in breast augmentation is, capsular contracture. This is formation of scar tissue around the implants. At times this scar tissue may become visible and require surgery to correct it. To reduce the risk of capsule formation Dr. Mabourakh will discuss techniques of massaging the implants after healing is complete as well as implant placement under the muscle.

There is also some thought that use of Vitamin E capsules may be beneficial.

Excessive bleeding following the operation is a rare problem , but it is one reason for re-operation to remove the accumulated blood to reduce pain and possible risk of capsule formation.

Infection is extremely rare. Everyone is placed on antibiotics following surgery to reduce this risk.

Breast sensation will change after breast enhancement. Most often reduction of sensation is reported. Some patients complain of increased sensation. Sensation usually improves over a period of months, but almost everyone has some sensation loss of the nipple areola complex.

There is no evidence that breast implants will affect fertility, pregnancy, or your ability to nurse. If, however, you have nursed a baby within the year before augmentation, you may produce milk for a few days after surgery. This may cause some discomfort, but can be treated with medication prescribed.

Leakage of breast implants is more common with saline implants than with today's silicone implants. Allergan and Mentor both have a lifetime warranty on the implants if there is a leakage issue. There is also a extended warranty available from the manufacturers that will cover the cost of replacement in the first 12 years after surgery.

Cancer detection is more difficult with implants in place. a displacement mammography is performed to try and visualize more of the breast tissue on x-rays. Even with this technique some of the breast is not seen.

Planning Your Surgery

In your initial consultation, Dr. Mabourakh will evaluate your health and explain which surgical techniques are most appropriate for you, based on the condition of your breasts and skin tone. If your breasts are sagging, we may also recommend a breast lift.

Be sure to discuss your expectations frankly with Dr. Mabourakh. He will be equally frank with you, describing your alternatives and the risks and limitations of each. You may want to ask Dr. Mabourakh for a copy of the manufacturer's insert that comes with the implant -- just so you are fully informed about it. And, be sure to tell Dr. Mabourakh if you smoke, and if you're taking any medications, vitamins, or other drugs.

Dr. Mabourakh will also explain the type of anesthesia to be used, the type of facility where the surgery will be performed, and the costs involved. Because most insurance companies do not consider breast augmentation to be medically necessary, carriers do not cover the cost of this procedure.

Preparing For Your Surgery

Dr. Mabourakh will give you instructions to prepare for surgery, including guidelines on eating and drinking, smoking, and taking or avoiding certain vitamins and medications.

While making preparations, be sure to arrange for someone to drive you home after your surgery and to help you out for a few days, if needed.

Where Your Surgery Will Be Performed

Dr. Mabourakh may prefer to perform the operation in our private fully approved surgery center or  a freestanding surgery center, or a hospital outpatient facility.

Types Of Anesthesia

Breast augmentation can be performed with a general anesthesia, so you'll sleep through the entire operation. Dr. Mabourakh may use a local anesthesia, combined with a sedative to make you drowsy, so you'll be relaxed but awake, and may feel some discomfort.

The Surgery

The method of inserting and positioning your implant will depend on your anatomy and Dr. Mabourakh 's recommendation. The incision can be made either in the crease where the breast meets the chest, around the areola (the dark skin surrounding the nipple), or in the armpit. Every effort will be made to assure that the incision is placed so resulting scars will be as inconspicuous as possible.

Working through the incision, Dr. Mabourakh will lift your breast tissue and skin to create a pocket, either directly behind the breast tissue or underneath your chest wall muscle (the pectoral muscle). The implants are then centered beneath your nipples.

Dr. Mabourakh believes that putting the implants behind your chest muscle may reduce the potential for wrinkling and capsular contracture. This placement may also interfere less with breast examination by mammogram than if the implant is placed directly behind the breast tissue. Placement behind the muscle however, may be more painful for a few days after surgery than placement directly under the breast tissue.

You'll want to discuss the pros and cons of these alternatives with Dr. Mabourakh before surgery to make sure you fully understand the implications of the procedure he recommends for you.

The surgery usually takes one to two hours to complete. Stitches are used to close the incisions, which may also be taped for greater support. A gauze bandage may be applied over your breasts to help with healing.

After Your Surgery

You're likely to feel tired and sore for a few days following your surgery, but you'll be up and around in within 24 hours. Most of your discomfort can be controlled by medication prescribed.

You will be placed in a surgical bra immediately after your surgery. Special tapes will be used to hold breasts in place and will be removed in five to seven days. You will be asked to wear the surgical bra exclusively for up to 2 months. You may also experience a burning sensation in your nipples for about two weeks, but this will subside as bruising fades.

There are no exposed stitches as Dr. Mabourakh uses only buried absorbable stitches, but the swelling in your breasts may take three to five weeks to disappear.

Getting Back To Normal

You will be able to drive in 3 to 5 days after surgery. You may also be ready for a desk type duty with in 3 to 5 days as well. Full recovery in 4 to 6 weeks.

During you post operative visits Dr. Mabourakh will review your restrictions and recommend activities you may start.

Your scars will be firm and pink for at least six weeks. Then they may remain the same size for several months, or even appear to widen. After several months, your scars will begin to fade, although they will never disappear completely.
